Maintain Root Causes
Purpose of application is to set up new or update existing root causes (e.g. theft, defective item), which are then utilized by Maintain Deficiencies application. For details, see Maintain Deficiencies section of this training guide.

  1. Open Maintain Root Causes application.
  2. Select a root cause from the drop down list and click .
  3. Review the General Information and Update Control sections of the form and make any necessary adjustments.
  4. When you are done, click .

Setting up a new root cause
  1. Open Maintain Root Causes application.
  2. Enter a 1-6 character abbreviation for root cause ID and click .
  3. Fill out the General Information and Update Control sections of the form.

    FORM FAQ
    General Information
    Status Current status of the root cause (e.g. active, inactive).
    Short Description Short description of the root cause can be entered here (e.g. design error, weather).
    Long Description A more thorough description can be entered here.
    Update Control
    Update Deficiency Root Cause info for divisions You can activate the root cause in one more divisions by selecting the check boxes.
  4. When you are done, click .
